§ 4.28.050. Application—Form.

A.

Printed forms of applications for absentee ballots in accordance with the requirements of this title shall be provided by the board and shall be available upon request to a registered voter. The board shall prescribe all forms and related affidavits and statements.

B.

On the application, the board may require the voter to furnish information from which eligibility may be verified.
